用黑色笔清晰填写或打印,回答所有问题.如果某个问题对您不适用,请填写“N/A”.如果对于某个问题的答案是“none”,请填写“none”.如果需要额外的空间去回答问题,请另附纸张.另附的纸张上,请写清:您的姓名,A#,和您回答的问题题号.提交申请的同时必须同时提交所有要求提供的原始证据.申请必须有正确的签名,和申请费.

I-140的申请地址，按照申请人的地址，依辖区分别寄到NSC或TSC.

如果受益人的排期当前，申请人的I-140和受益人的I-485（还有附属的I-131/I-765）可以同时提交，目前EB-1类总是如此。这时Concurrent

Filing的地址，NSC有些不同。
